HealthPathways is an online portal that provides GPs and other health professionals with easy access to comprehensive, evidence-based assessment, management and localised referral resources for specific health conditions.
Working in partnership, SA Health, Adelaide PHN and Country SA PHN, are implementing HealthPathways across South Australia. HealthPathways will support consistent care by using a standardised approach to the management of health conditions.

The HealthPathways online portal provides a range of condition-specific pathways. GPs and other health professionals across the health sectors will collaborate on the development and implementation of local pathways to ensure patients receive the right care in the right place at the right time. HealthPathways is designed to be used at the point of care by general practitioners but is also available to specialists, nurses, and allied health professionals across South Australia. Tailored to the local context, HealthPathways supports health services to ensure patients receive consistent, accessible, quality and timely care.
HealthPathways South Australia will support the consistent management of health conditions and improve the health outcomes and journeys of all South Australian’s through our local health system.
HealthPathways South Australia (SA) is a partnership between SA Health, Adelaide Primary Health Network and Country SA Primary Health Network. General Practitioners, hospital specialists, nurses, allied and other health professionals will be engaged to collaborate on the localisation of pathways. The partners have established a team to implement HealthPathways South Australia. The HealthPathways team includes GP Clinical Editors, Clinical Coordinators and Clinical Leaders.
HealthPathways South Australia will facilitate communication and collaboration between health professionals across primary and acute care settings and encourage a greater understanding between the sectors to improve the journeys of all South Australians through our local health system.
The HealthPathways team is keen to hear from General Practitioners, hospital specialists, nurses, allied and other health professionals who are interested in being involved.
