Health Care Home stage one application round now open

On Friday 4th November, Minister Sussan Ley announced that medical practices, including Aboriginal Community Controlled Health Organisations and Aboriginal Health Services, in the 10 PHN trial sites can apply for stage one of Health Care Homes.

To apply, a general practice or ACCHO must:
-    Be located in one of these ten PHN regions
-    Meet the eligibility and assessment criteria set out in the application form and guidelines.

The ten stage one PHN trial sites are: Perth North; Northern Territory; Adelaide; Country South Australia; Brisbane North; Western Sydney; Nepean Blue Mountains; Hunter, New England and Central Coast; South Eastern Melbourne; and Tasmania.

Applications close Thursday 15 December 2016.

Following the selection process, Country SA PHN will be providing a range of support to successful applicants to achieve readiness for implementation of the Health Care Home on 1 July 2017.
